ABOUT OFFICE-BASED ANESTHESIA
The board-certified anesthesiologists
and registered nurses of VAC Mobile Services, a division of Valley Anesthesiology
Consultants, are delighted to provide anesthesia for office-based procedures. Valley
Anesthesiology Consultants is one of the nation’s premier providers of anesthesia
services; a physician group nationally recognized for its commitment to the highest
standards of patient care, serving patients and their physicians throughout Arizona,
for more than 28 years. VAC Mobile Services maintains the accreditation standards
of the Accreditation Association of Ambulatory Health Care (AAAHC).

To promote and ensure high quality and efficient office-based
anesthesia services for surgeons/proceduralists and their patients. Valley is committed

PATIENT RIGHTS AND RESPONSIBILITIES
This notice describes your rights and responsibilities
concerning your medical care. Please review it carefully and let us know if you
have questions. Except where medically contraindicated, these rights apply to all
adults, children, and adolescents treated by VAC Mobile Services and their parents
and/or guardians.
EACH PATIENT HAS THE RIGHT…
- To access medical care
- To have access to an interpreter,
- To be treated with respect, consideration, and dignity,
- To receive a written statement of his/her rights
and responsibilities as a patient,
- To know the identity and professional status of
his/her caregivers,
- To receive an informed consent prior to the procedure,
or to refuse treatment,
- To privacy and security,
- To understand disclosures and records are treated
confidentially, and to approve or refuse their release, except when release is required
by law,
- To participate in his/her health care decisions,
- To consent to or refuse to participate in research
or educational projects,
- To receive truthful information regarding his/her
illness along with knowledge of the benefits and risks of each alternative treatment
- To understand provisions for after-hours and emergency
care,
- To understand the fees for services and payment
policies, and to request an itemized bill,
- To receive information regarding methods for expressing
suggestions or grievances to the organization,
- To request a different anesthesia provider.
PATIENTS ARE RESPONSIBLE FOR…
- Keeping appointments or notifying the healthcare
provider when unable to do so,
- Reporting complete and accurate information about
matters relating to their health, including prescription and over-the-counter medications
and dietary supplements, and any allergies or sensitivities,
- Reporting whether they understand the proposed treatment
and what is expected of them,
- Following instructions of agreed upon treatment
plans,
- The consequences of their actions if they refuse
treatment or do not follow instructions,
- Providing a responsible adult to transport him/her
home from the facility and remain with him/her for 24 hours, if required by his/her
provider,
- Assuring that financial obligations for their healthcare
are fulfilled,
- Being considerate of the rights of other patients
and personnel,
- Being Respectful of the property of Valley Mobile
Services.
POTENTIAL CONFLICT OF RIGHTS
Where any person raises a concern that remains unresolved
regarding a divergence of opinion regarding the rights or treatment of a child,
or adolescent patient and the rights of their parents and/or guardians, the Valley
Mobile Services shall consult with the Arizona Department of Child Protective Services
to ensure that the minor’s rights are protected.
HOW TO FILE A COMPLAINT OR GRIEVANCE
Any patient or patient’s representative that has a concern
with Valley Mobile Services may submit a written request for resolution to:

WHAT IS OFFICE-BASED ANESTHESIA?
Sometimes referred to as ambulatory anesthesia, special
medications and techniques are used when a patient is expected to go home as soon
as possible on the same day of the surgery. Office-based surgery is a type of ambulatory
anesthesia.
Since most patients want to avoid an overnight hospital
stay after surgery, ambulatory surgical procedures have become very popular. There
has been a growing trend for surgery to be performed in a physician’s office. Office-based
surgery can offer the convenience of having a procedure performed in a more comfortable
setting and with a quick return home.

OFFICE-BASED ANESTHESIA FEES
For questions regarding office-based anesthesia billing,
please contact 602.744.4760. For dental and some plastic surgery
cases, payment is required in advance of the procedure. A representative of Valley
Anesthesiology Consultants’ billing service will contact the patient (or responsible
party) after the procedure is scheduled to provide the patient with the estimated
anesthesia fee and collect payment via credit card. This pre-payment is based on
the estimated procedure time; however, the final dental anesthesia fee will be based
on the precise duration of the anesthesia services. Payment by credit card (VISA,
MasterCard, American Express, or Discover) will be required at this time.
The patient will be refunded in the amount of any over-payment.
